In a post apocalyptic woodland area, Malorie Shannon advises two young unnamed children that they are going downstream on a river in a boat while giving strict rules on not to remove their blindfold fabrics.
5 years earlier, a pregnant Malorie is doing a routine check on her soon-to-be-born baby with her older sister Jessica. While on her way to join Jessica, Malorie eyes a woman bashing her head into a glass panel followed by others beginning to panic as chaos arises in the city.
As Malorie and Jessica drive away from the chaos, Jessica begins to tear up and loses control of her driving. Despite Malorie trying to stop her from crashing, they head up overturning the car. Malorie is shocked and horrified when she witnesses an injured Jessica begin to convulse and die from a seizure, devastating her.
A group of survivors rescue Malorie and while recovering at their base, she learns from one of the survivors, Charlie, that an otherworldly entity has invaded Earth, taking the form of it’s victims worst fears and driving them insane before causing them to commit suicide. At the instance of Douglas, the leader of the group, they should be blindfolded at all times when they collect supplies or search for survivors.
Malorie, along with a survivor named Olympia, who is also pregnant, give birth to two children. While Malorie survives, Olympia dies from childbirth that leads to Malorie adopting the baby girl. Malorie doesn’t give the girl and her newborn son names until she and the survivors try and find a safe haven.
When on a search for rations, Malorie and the group enter an abandoned supermarket but Charlie accidentally removes his blindfolded and commits suicide as a result. The entity rampages through the supermarket, separating Malorie from the others. Upon returning back to the base, Malorie finds out that a survivor is intending to take the children but is stopped by Felix, another survivor, who fights with the would-be kidnapper while Malorie flees with the babies. Malorie is also forced to be separated from Tom, a survivor she recently fell in love with.
Alone in the woods with her children at her side, Malorie begins training the children on how to embrace the apocalypse. During the day they travel down the river, Malorie comes face to face with a river hermit who almost kills her but she is able to drown him. Upon reaching land, the entity senses her and the children, giving chase when the trio runs for their lives until they come to a clearing.
While catching her breath, the boy removes his blindfold to Malorie’s horror but it turns out he along with the girl are immune to the entity, forcing it to leave them alone.
Malorie and the children reach the windowless safe haven which she learned about while listening to a radio announcement and reunite with Tom, who survived the incident at the base, and Malorie learns that everyone at the base have deliberately blinded themselves to avoid seeing the entity. The film ends as Malorie, Tom and the newly-named children adapting to the new life inside the safe haven.
